How it works: It belongs to a group of medicines called organic nitrates. Organic nitrates work by widening the blood vessels in your heart to allow an increased amount of blood to flow to areas which need it.

Active ingredient: Isosorbide Dinitrate.

Elderly should be given lowest recommended daily dose initially then titrate upward. Angina : Adults : 5-40mg 4 times/day or 40mg every 8-12hr in sustained-release dosage form. Congestive heart failure : Adults : Initial dose 10mg 3 times daily;target dose 40mg 3 times daily; max dose 80mg 3 times daily. Sublingual tab : 2.5-10mg every 4-6hr. Children : Not recommended. OR As directed by your physician.

Do not take if: • You are allergic to isosorbide dinitrate, other nitrates or any of the other ingredients of this medicine . • You suffer from anaemia (reduction in red blood cells which can make the skin pale and cause • weakness or breathlessness) • You have had a heart attack (myocardial infarction) • You have had a brain haemorrhage (bleeding) • You have had a head injury (trauma) • You have a low blood volume (hypovolaemia) • You have very low blood pressure .
Blood pressure and pulse rate should always be monitored and the dose adjusted according to the patient's response.
Talk to your doctor or pharmacist before taking if You have an abnormally low level of oxygen in the blood and impaired gas exchange due to lung disease or ischaemic heart disease .
Talk to your doctor or pharmacist before taking if you have been diagnosed with any of the following heart conditions: hypertrophic obstructive cardiomyopathy (HOCM), constrictive pericarditis, cardiac tamponade, low cardiac filling pressures, aortic/mitral valve stenosis and diseases associated with raised intra-cranial (head) pressure .
The development of tolerance (decrease in efficacy) as well as cross tolerance towards other nitrate-type drugs (decrease in effect in case of a prior therapy with another nitrate drug) has been described. For a decrease in, or loss of, effect to be prevented, continuously high dosages must be avoided.
