What it is used for: Depression.
Indications as listed by the manufacturer: Depressive Episodes Associated with Bipolar I Disorder : It is indicated for the acute treatment of depressive episodes associated with Bipolar I Disorder in adults ; Treatment Resistant Depression : It is indicated for the acute treatment of treatment resistant depression (Major Depressive Disorder in adults who do not respond to 2 separate trials of different antidepressants of adequate dose and duration in the current episode).
How it works: Fluoxetine: Inhibits CNS neuron setotonin reuptake with minimal or no effect on norepinephrine or dopamine reuptake; does not bind to alpha-adrenergic, histamine, or cholinergic receptors Olanzapine: Potent antagonist of dopamine 1-3, histamine H1, serotonin 5-HT2A and 5-HT2C, and alpha1-adrenergic receptors; shows moterate antagonism for 5-HT3, and muscarinic M1-5 receptors; binds to GABA-A, benzodiazepine, and beta-adrenergic receptors .
Active ingredient: Olanzapine , Fluoxetine.
Reported side effects: Chills, neck rigidity, photosensitivity reaction ,vasodilatation , QT-interval prolonged , diarrhea , gastritis, gastroenteritis, nausea and vomiting, peptic ulcer , gastrointestinal hemorrhage, intestinal obstruction, liver fatty deposit, pancreatitis , ecchymosis , anemia, thrombocytopenia , leukopenia, purpura , generalized edema, weight loss , bilirubinemia, creatinine increased, gout , osteoporosis .
Known drug interactions: Monoamine Oxidase Inhibitors (MAOI) , CNS Acting Drugs , Serotonergic Drugs , Triptans , Tryptophan , Drugs that Interfere with Hemostasis (e.g., NSAIDs, Aspirin, Warfarin) , Benzodiazepines , Inducers of 1A2 , Inhibitors of CYP1A2 , Pimozide , Carbamazepine , Thioridazine , Tricyclic Antidepressants (TCAs) , Antihypertensive Agents , Levodopa and Dopamine Agonists , Benzodiazepines , Clozapine , Haloperidol , Phenytoin , Lithium , Drugs Tightly Bound to Plasma Proteins , Valproate , Biperiden , Theophylline .

Depression Associated with Bipolar I Disorder : Initial: 25 mg/6 mg orally daily in evening . If needed, may titrate with 25-50 mg fluoxetine/6-12 mg olanzapine; not to exceed 75 mg/18 mg per day . Treatment of Resistant Depression : Initial: 25 mg/6 mg orally daily in evening . If needed, may titrate with 25-50 mg fluoxetine/6-12 mg olanzapine; not to exceed 75 mg/18 mg per day . Hypotensive diathesis, hepatic impairment, or slow metabolism: Initiate with 25 mg/3 mg to 25 mg/6 mg orally every day . OR As directed by your physician.

- The use is contraindicated with the following: • Monoamine Oxidase Inhibitors (MAOI) • Pimozide • Thioridazine • Hypersensitive to any of the ingredients.
- Consideration should be given to changing the therapeutic regimen, including possibly discontinuing the medication, in patients whose depression is persistently worse, or who are experiencing emergent suicidality or symptoms that might be precursors to worsening depression or suicidality, especially if these symptoms are severe, abrupt in onset, or were not part of the patient’s presenting symptoms .
- Patients with Major Depressive Disorder (MDD), both adult and pediatric, may experience worsening of their depression and/or the emergence of suicidal ideation and behavior (suicidality) or unusual changes in behavior, whether or not they are taking antidepressant medications, and this risk may persist until significant remission occurs.
- It is unknown whether the suicidality risk extends to longer-term use, i.e., beyond several months. However, there is substantial evidence from placebo-controlled maintenance trials in adults with depression that the use of antidepressants can delay the recurrence of depression.
- All patients being treated with antidepressants for any indication should be monitored appropriately and observed closely for clinical worsening, suicidality, and unusual changes in behavior, especially during the initial few months of a course of drug therapy, or at times of dose changes, either increases or decreases.
