What it is used for: Schizophrenia.

How it works: The mechanism of action of aripiprazole in schizophrenia or bipolar mania, is unknown. However, the efficacy of aripiprazole could be mediated through a combination of partial agonist activity at D2 and 5-HT1A receptors and antagonist activity at 5-HT2A receptors. Actions at receptors other than D2, 5-HT1A, and 5-HT2A may explain some of the other clinical effects of aripiprazole (e.g., the orthostatic hypotension observed with aripiprazole may be explained by its antagonist activity at adrenergic alpha1 receptors).

Active ingredient: Aripiprazole.

Reported side effects: Insomnia , restlessness , headache , dizziness , akathisia , somnolence , tremor , blurred vision , GI upset , asthaenia , Stroke , TIA in elderly demented patients , diabetes , anxiety , sedation , cardiac rhythm disorders , dysphagia.

Known drug interactions: Potent inhibitors of CYP3A4 or 2D6 . Potent inducers of CYP3A4 . Alpha blockers , alcohol , CNS drugs. Drugs that prolong QT interval , other antihypertensives , serotonergic agents , stimulants.

Adults : Schizophrenia : Initial /Target : 10-15mg daily . Titrate : Should not increase before 2 wk. Maintenance : Periodically reassess need for therapy . Adjust dose with CYP3A4 inducers or inhibitors , or with CYP2D6 inhibitors . Bipolar Disorder : Acute Manic or Mixed Episodes : Initial : 30mg daily . Titrate : May decrease to 15mg every day based on tolerability . Adjust dose with CYP3A4 inducers or inhibitors , or with CYP2D6 inhibitors . Children : Treatment with mania : 13-18 yr , initially 2mg once daily for 2 days , increased to 5mg once daily for 2 days, thereafter increased to usual dose of 10 mg once daily ; further increased if necessary in steps of 5mg to max 30mg daily , max duration of treatment 12wk. Schizophrenia : 15-18yr , initially 2 mg once daily for 2 days , increased to 5mg once daily for 2 days , thereafter increased to 10mg once daily ; further increased if necessary if necessary in steps of 5mg to max 30mg daily. OR As directed by your physician.
