What it is used for: Fungal Infection.

Indications as listed by the manufacturer: Treatment of invasive aspergillosis, Treatment of candidaemia in non-neutropenic patients, Treatment of fluconazole-resistant serious invasive Candida infections (including C. krusei)., Treatment of serious fungal infections caused by Scedosporium spp. and Fusarium.

How it works: The mechanism of action of voriconazole is inhibition of cytochrome P450 (CYP 450)–dependent 14a-lanosterol demethylation, which is a vital step in cell membrane ergosterol synthesis by fungi. For yeasts, voriconazole appears to be fungistatic, as are other azoles.

Active ingredient: Voriconazole.

Reported side effects: The most common side effects are nausea, altered bowel habit (constipation, diarrhoea, or flatulence) abdominal pain, and visual disturbances. If you get visual disturbances these usually get better over a few days or weeks and do not damage your eyes. Some patients experience troubling dreams, poor concentration or unsteadiness standing or walking. Voriconazole can cause peripheral neuropathy.

Known drug interactions: Medications can affect the removal of voriconazole from your body, which may affect how voriconazole works. Examples include butalbital, efavirenz, mitotane, rifamycins (such as rifabutin, rifampin), ritonavir, secobarbital, certain drugs used to treat seizures (such as carbamazepine, phenobarbital).

Adults : For first 24hr : 400mg every 12hr (>40kg) or 200mg every 12hr (<40kg) , Maint: 200mg twice daily (>than 40kg) or 100mg twice daily (<40kg). If response inadequate it may be increased to 300mg twice daily (>40kg) or 150mg twice daily (<40kg) . A tablet should be taken 1 hr before or 1hr after food . Children : Under 2yr : Not recommended ; 2-12yr or 12-14yr and body wt <50kg : Use Infusion until significant improvement then 9mg/kg twice daily . (preferred as susp <12yr) . Titrate if necessary in 1mg/kg increments (50mg increments if initial dose was 350mg); max 350mg twice daily ; 12-14yr and body wt<50kg , same as adults . Tablet should be taken 1hr before and 1hr after food ; susp should be taken 1hr before or 2hr after food OR As directed by your physicians .
