What it is used for: Antidote.
Indications as listed by the manufacturer: It is used in cancer treatment (cytotoxic therapy) as an antidote to reduce the toxicity (poisonousness) and effect of certain cancer drugs, the so-called folic acid antagonists (e.g. methotrexate).
How it works: Leucovorin is folic acid in its active (reduced) form, so it allows nucleic acid synthesis to proceed even in the presence of methotrexate. Leucovorin can also compete with methotrexate for the same transport processes into the cell .
Active ingredient: Leucovorin Calcium.
Reported side effects: Uncommon (may affect up to 1 in 100 people) : * Fever Rare (may affect up to 1 in 1,000 people) * Insomnia, restlessness and depression after high doses * Increase in the frequency of fits in epileptics * Stomach and intestinal complaints (gastrointestinal complaints) after high doses Very rare (may affect up to 1 in 10,000 people) : * Allergic (anaphylactic and anaphylactoid) reactions and hives (urticaria), ranging up to lifethreatening shock.
Known drug interactions: 5-fluorouracil , Methotrexate , folic acid antagonist (e.g. cotrimoxazole, pyrimethamine), phenobarbital, primidone, phenytoin and succinimide.

Antidote : To be adjusted to individual needs , provided that it corresponds to approximately 10 to 20% of the methotrexate dose . Megaloblastic anaemias : Treatment , initial course of 10 mg daily for 10 to 15 days , but 1mg daily is probably adequate , subsequent therapy and dosage modified to the haematological response . OR As directed by your physician .
Active ingredient: Leucovorin Calcium. Labelled strength: 50mg.
- Do not use Leucovorin: If you are allergic to calcium folinate or any of the other ingredients of this medicine . - In pernicious anaemia or other forms of megaloblastic anaemia caused by vitamin B12 deficiency. While improvements in blood count may occur, the nervous system signs continue to progress. This obscures the pernicious anaemia, making it more difficult for the doctor to detect it .
- If you suffer from a cancer which is being treated with 5-fluorouracil (e.g. cancer of the bowel): While Leucovorin can be used together with 5-fluorouracil in cytotoxic cancer treatment, it has been shown that, when they are used at the same time, this increases the efficacy and toxicity of 5-fluorouracil .
- If you suffer from epilepsy and are being treated for it: In epileptics who are being treated with the active substances phenobarbital, phenytoin, primidone and succinimides, there is a risk that the frequency of epileptic fits will increase, as Leucovorin reduces the concentration of these drugs in the blood.
- Leucovorin should only be used together with the cancer drugs methotrexate or 5-fluorouracil under the direct supervision of a doctor who has experience with the use of chemotherapeutic agents in cancer.
- If you suffer from a cancer of the central nervous system: Leucovorin may only be given as in injection into muscle (intramuscularly) or into a vein (intravenously) and must not be used in the central nervous system (intrathecally). Deaths have been reported after the intrathecal administration of folinic acid (the active substance of Leucovorin) following a prior intrathecal overdose of the cancer drug methotrexate
