What it is used for: Hypertension.
Indications as listed by the manufacturer: Treatment of essential hypertension . Patients whose blood pressure is not adequately controlled on irbesartan or amlodipine monotherapy .
How it works: Irbesartan : Irbesartan is a potent, orally active, selective angiotensin-II receptor (type AT1) antagonist. It is expected to block all actions of angiotensin-II mediated by the AT1 receptor, regardless of the source or route of synthesis of angiotensin-II. The selective antagonism of the angiotensin-II (AT1) receptors results in increases in plasma renin levels and angiotensin-II levels, and a decrease in plasma aldosterone concentration. Serum potassium levels are not significantly affected by irbesartan alone at the recommended doses. Irbesartan does not inhibit ACE (kininase-II), an enzyme which generates angiotensin-II and also degrades bradykinin into inactive metabolites. Irbesartan does not require metabolic activation for its activity. Amlodipine : The mechanism of the antihypertensive action of amlodipine is due to direct relaxant effect on vascular smooth muscle. The precise mechanism by which amlodipine relieves angina has not been fully determined but amlodipine reduces total ischemic burden by the following two actions. Amlodipine dilates peripheral arterioles thus, reduces the total peripheral resistance (after load) against which the heart works.
Active ingredient: Irbesartan and Amlodipine.
Reported side effects: Headache , drowsiness , dizziness , fatigue , oedema , nausea , visual disturbances , abdominal pain , flushing , dyspnoea , altered bowel habits .
Known drug interactions: Aliskiren in patients with diabetes or with moderate to severe renal impairement . ACEIs.

- Hypersensitive to any of the ingredients in the list .
- As for any patient receiving diuretic therapy, periodic determination of serum electrolytes should be performed at appropriate intervals
- There is an increased risk of severe hypotension and renal insufficiency when patients with bilateral renal artery stenosis or stenosis of the artery to a single functioning kidney are treated with angiotensin converting enzyme inhibitors or angiotensin-II receptor antagonists. While this is not documented with Irbesartan/Hydrochlorothiazide , a similar effect should be anticipated.
- Safety of Amlodipine Besylate in human pregnancy or lactation has not been established.
- Amlodipine Besylate used at similar doses in elderly or younger patients, is equally well tolerated. Therefore, normal dosage regimens are recommended.
